    Our team reviews all incoming referrals and inquiries to ensure they meet our specialized criteria.

  • 14 Days

    2. Scheduling:

    We will contact you (or your patient) within 14 days to confirm the referral has been received and to schedule a comprehensive assessment.

  • After Assessment

    3. The Roadmap:

    Following the assessment, a detailed “Model of Care” report is generated and shared with the primary care provider.

Not What You’re Looking For?

To maintain our focus as a Center of Excellence for the Shoulder and Elbow, we do not provide primary assessment for the Hip, Knee, or Low Back.

  • For Hip & Knee concerns: Please contact your GP to request a referral to the Ontario Rapid Access Clinics (RAC).
  • For Low Back & Spine concerns: Please visit the Ontario RAC Low Back Program.
